I had similar happen with a hardware failure -- one of the disks on the RAID was reporing bad sectors (now, why wouldn't the idiotic Adaptec RAID hardware controller remap them automagically and just keep on chugging along?!?!?!?!?!!?!?!?!?1111111). I ended up with duplicate rows of the same PK on some table. I had to rebuild the whole node from scratch, since I couldn't prove the data was identical to the origin, and it changes so fast.
