The UPDATE FUNCTIONS upgrade from a 2.1.x (or 2.0.x) Slony to 2.2.0 does not create the no_failed column in sl_node.

Proposed fix 

Create the no_failed column (if required) during the updateSchema function call

Passed regression tests.

Failed two (clone node) tests from disorder, but should be irrelevant.

Applied to master and REL_2_2_STABLE.
